QUARTERLY REPORT ON ASSESSMENT KS1, KS2, AND KS3 TEMPLATE
Every end of the quarter, DepEd teachers, particularly the advisers, are required to submit their quarterly report on assessment which indicates the ratings of their students over one grading period. This has to be encoded in a template to be forwarded to the ICT coordinator or to the personnel tasked to consolidate all the data.
Inside the QRA form, teacher concerned has to enter in the School Information Dashboard the following data needed for: School Year, Quarter, District, School Name, School Id, Grade Level, Section, Teacher/Adviser, School Testing Coordinator, School Head/Principal, and Public School District Supervisor.
Inside the QRA Form 1, you need to enter the names of the students and their corresponding grades per subject. In QRA Form 2, there appears the consolidated analysis of progress and achievement report per grade. Here, students' grades are being categorized according to the level of proficiency to wit: Outstanding (90-100), Very Satisfactory (86-89), Satisfactory (80-84), Fairly Satisfactory (75-79, and Did Not Meet Expectation (Below 75). Inside the QRA Form 3 is a consolidation which is automatically generated from QRA FORM 2.
